Nothing to do with software - it's the posters. It's caused by people misusing their backspace button and deleting the right hand bracket on someone they're quoting... subsequent posters who quote these posts just compound the mess.

I used to edit the bracket back in on threads where the issue arose - nowadays I just do it on threads that have some value.
